Anonymous feedback
Although I prefer in-person feedback whenever possible (conversation tends to inspire a deeper level of mutual understanding), I understand that it can be uncomfortable to give direct feedback on certain subjects. To this end, I've set up an anonymous feedback form. I'm interested in both positive and negative comments on any aspect of the course (lectures, office hours, TAs, assignments, material, etc.). You can also use this form to anonymously ask questions about the material, although I would strongly encourage you to ask questions in class instead (if you're confused, it's likely some of your classmates are, too).
