Student Information Form

Please complete all sections of the Student Information Form in PeopleSoft

  • verify/update your Social Security Number and birthdate
  • indicate your interest in pre-health (medical, dental, veterinary) or pre-engineering programs
  • verify/update your home address, home phone and cell phone
  • add your parents' names and address

SSN and birthdate - these are important elements for enrollment verifications to loan and insurance agencies (and eventually to prospective employers) and required for the 1098T tax forms. Please review our data and update if necessary. If you do not have a U.S. social security number, leave the information blank.

Interest in pre-professional programs - if you check either pre-health professions or pre-engineering, you will receive campus mailings from the pre-professional program advisors. There will be information for these programs included in the First Days academic information sessions.

Verify/Update your home address and phone numbers

  • if you need to change your home address, use the Campus Personal Information > Addresses links and update the information.
  • if you need to change your home telephone number, use the Campus Personal Information > Phones links and update the information. Your home telephone number should in most cases be a shared number with your parents.
  • if you need to add or update your cell phone number, use the Campus Personal Information > Phones links and update the information.

Parent/Guardian Information

  • please ask your parents how they wish to be addressed in college mailings - e.g. Mr. & Mrs. John Doe, Jane & John Doe, Dr. Jane Smith & Mr. John Doe. Enter this on the Student Information Form.
  • 'same address as student's home address' will be checked by default. If your home address is not the same as your parent or guardian, uncheck the 'same as' box and enter the correct address. If the 'same as' box is checked, any time you update your own address and home phone, your parents' address and phone will also update. The phone number should be a shared home phone number if the address is the same as yours.

Non- or Co-Custodial Parent Information

  • you only need to enter information if you have a non- or co-custodial parent who should also receive any college mailings (including financial aid mailings or billing). If you have a non-custodial parent who will not be involved in your Williams education, you do not need to provide this information.
  • ask your parent how they wish to be addressed in college mailings (this may include step-parents).
  • add names and a mailing address

Please add Emergency Contact information for each of your parents under the Campus Personal Information > Emergency Contact link. You should include each of your parents and include a home phone number as well as any cell or work phone numbers for each. Remember to update this information whenever necessary during your time at Williams. Check the instructions for adding emergency contact information if you aren't sure how to add this information.
